The purpose of this role is to provide support and assistance to Aboriginal and Torres Strait Islander consumers within South West Health region, delivering flexible, accessible and holistic health care that incorporates a multi-disciplinary, collaborative care planning approach to meet individual and family needs. This position may require the incumbent to travel between various facilities within the SWHHS to deliver services and some travel may require overnight stays.
