About the Opportunity
The position is responsible for the provision of culturally responsive, client centred care coordination service to Aboriginal families with children aged 0-5 within a Sustained Health Home visiting model.

The position will work collaboratively with Child and Family Health Nurses and other allied health professionals to provide assessment, care coordination and education to families and take into account clinical, psychosocial and cultural needs.

  • Certificate III in Aboriginal & Torres Strait Islander Primary Health Care or relevant qualification OR willingness to complete and to undertake further training as identified.
  • Demonstrated understanding of the range of factors that have influenced the health status of Aboriginal children and families.
  • Demonstrated experience delivering culturally responsive health services to Aboriginal communities.
  • Ability to undertake health/social needs assessments that demonstrate a comprehensive understanding of Aboriginal culture and social issues in relation to health matters; and make referrals to community service providers.
  • Demonstrated experience working effectively as part of a multidisciplinary team to provide care coordination or case management services.
  • Demonstrated written and verbal communication skills and computer literacy.
  • Current unrestricted Australian drivers licence (P2 Licence Acceptable) subject to obtaining NSW drivers licence within 3 months of appointment.
