ACAT Assessor - Registered Nurse/ Allied Health Assistant

About the role

The ACAT Assessor position stands within the of the CCH/ACAT multidisciplinary team. The primary purpose of the position is to provide holistic comprehensive assessment, support and management to the target population in the St George Geographical area in the community, hospitals and aged care facilities as applicable.

Key Responsibilities

  • ACAT assessments completed within nominated KPI
  • 100% accurate documentation in the My Aged Care system and the electronic medical record
  • Maintaining CPD points
  • Completion of mandatory training

What you bring 

  • Qualifications in Nursing and eligibility for membership of the appropriate professional body and/or current registration with the Australian Health Practitioners Regulation Agency (AHPRA) and current unrestricted Class C driver’s licence.
  • Minimum of 3 years post graduate experience working with older people in an acute, aged care and/or community setting.
  • Knowledge of Aged Care Systems as well as Aged Care issues and experience working with clients from diverse cultural backgrounds.
  • Demonstrated ability to provide education to clients, peers and carers as well as commitment to ongoing self-education and best practice.
  • Demonstrated high level of interpersonal, written and verbal communication skills including computer literacy.
  • Demonstrated ability to work autonomously and as part of a multidisciplinary team.
  • Demonstrated knowledge of Quality Improvement initiatives and Work Health Safety principles relative to community service delivery.
  • A desire to join a team committed to the care of others in a mission based organisation.
