About the role:
Working solely on the Commonwealth Home Support Program (CHSP), this part-time role will complete in-home assessments to ensure our services meet the needs of our new or existing customers around the Nepean/Blue Mountains areas.
You will work in partnership with the office-based CHSP Coordinators to communicate customer needs and goals into efficient care plans that are delivered by our Home Care Workers.
To ensure we deliver quality services you will need to build strong relationships with customers and their families, identify service upgrade opportunities, ensure all information in the system is accurate and to ensure all services meet our compliance requirements.
This is a part-time permanent role (Hours are roughly 8:30am-5pm, 3 days per week). It can be any 3 days of your choosing but they must be consistent each week.
About you:
- Certificate III in Aged Care, or previous experience in Aged Care, highly regarded
- A strong understanding of Commonwealth Home Support Program (CHSP) is preferred but not essential
- Previous experience using customer management systems is preferred.
- A strong, professional phone manner
- Demonstrate understanding of safety practices and compliance principles
- Excellent ability to build rapport with customers and team members
- Exceptional time management, resilience and communication skills
- Medium computer skills including Google, Word, and Excel
- High attention to detail and an ability to work under pressure
- Reliable vehicle and valid driver’s licence
