Position Overview
As an Assistant Support Facilitator your role will be to primarily support the Real Community Service’s Facilitators across South-East Qld in an administrative capacity. Your role contributes greatly to the success of delivering timely, sustainable, and person-centered supports to people living with a disability so that they can achieve their personal goals and maintain socially valued roles, friendships and relationships. This newly created position is a fantastic opportunity to grow your career, gain valuable experience, and make a real impact on the lives of those we serve. This will be an Office based role at our Maroochydore Office with travel often required as far down as the Gold Coast. You will be well supported by the Regional Manager and ongoing training will be provided to the successful candidate.
Key Responsibilities
• Assisting Facilitators with the development and maintenance of Participant profiles, ensuring that compliance is met in alignment to the NDIS Practice Standards.
• Collaborate with Facilitators to assist in the development and implementation of personalised support plans for Participants.
• Support Facilitators in assessing and responding to risks, to ensure the safety and wellbeing of Participants and Staff.
• Monitor and document Participants' progress including communication, providing feedback and suggestions for improvements.
• Daily administrative tasks such as answering of phones, collecting documents from SILS, collating and file noting.
• Assist in the Co-ordination of daily activities and ensuring all support services are delivered effectively.
• Help maintain a positive and welcoming environment for Participants and Staff.
• Participate in team meetings and contribute to the continuous improvement of our services.

• Demonstrated knowledge of the National Disability Insurance Scheme and what it means for participants
• Excellent communication and interpersonal skills.
• Dependable knowledge in computer applications and numeracy skills
• Strong organisational abilities and attention to detail.
• Ability to work autonomously and collaboratively within a team environment.
• Flexibility and adaptability to meet the diverse needs of our participants.
• Previous experience in a support or caregiving role is an asset but not required.
• Relevant Qualifications in Disability, or Health Care are highly desirable but not essential.
Mandatory Requirements
• Current Blue and Yellow Card/NDIS Card
• NDIS Worker Orientation Certificate
• Valid QLD driver’s licence
• First Aid and CPR
• Qualifications within Disability or Health Care sector are highly regarded but not essential.
• Relevant experience in Case Management is highly regarded but not essential.
