Due to internal progression within the organisation, we are seeking applications from suitably qualified Care Coordinators in Port Macquarie.
About the Position:
Our My Aged Care Package Care Coordinator plays a critical role in supporting participants and their families to build capacity to coordinate their funded plans and navigate/negotiate appropriate support and services.
The successful applicant will commence as soon as practicable on a full-time permanent basis as part of the Kirinari Support 4 Me Team that operates across NSW and VIC.
About you:
- Experience and/or strong understanding of coordinating support services for people with varying needs in Aged and Home Care (including those with additional high or complex needs)
- A Qualification in Nursing and/or Health is highly regarded
- Ability to understand, budget and manage services within a funded package
- Strong written, verbal and interpersonal communication skills
- Strong time management and prioritisation skills
- Ability to work creatively and collaboratively to problem solve
- Current Driver's License
- Ability to obtain a NDIS Workers Screening Check Clearance
- A team player who is also capable of working autonomously
- Able to establish a connection with our customers and build a solid professional relationship
- Able to self motivate and motivate others
Permanent part time position; could negotiate full time hours for the right candidate.
