Position Details
Grampians Health Stawell Community Services is seeking to appoint highly motivated experienced Registered Nurses to join our Care Coordination team. The position offers the opportunity to learn and work in community programs.
The Care coordinator works with consumers to promote health independence and positive approaches to recovery from illness and managing chronic health conditions as per the Health Independence (HIP) Guidelines.
The role provides integrated and coordinated person centred care to improve client health outcomes and reduce hospital length of stay and avoidable admissions.

About You
The preferred applicant would have a strong interest in working with people in the community and with clients that have complex health care needs and chronic conditions.
The role would suit a self-motivated, team focused individual with a commitment to providing high quality holistic care to clients in community programs.
Knowledge of chronic disease management and demonstrated experience of effective communication skills with healthcare providers will be valued.
