Due to organisational growth, we are seeking experienced Care Finder Support Workers to join our Anglicare team of professionals.
About the Role:
Reporting to the Care Finder Team Leader to provide specialist and intensive assistance to help people within the care finder target population to understand and access aged care and connect with other relevant supports in the communities of Lithgow, Upper Mountains and Lower Mountains.
Key Duties:
Assertive outreach by engaging and building rapport with potential clients and local intermediaries, including outreach activities to target groups and target locations.
Partnerships are developed and maintained with key professionals to support the identification and engagement of the target groups.
Ensure general enquiries and intake applications are actioned in a timely manner.
Clients have access to services by supporting them to interact with My Aged Care for registration, eligibility for services, and referral for assessment, which will take place of client’s choosing, either face to face or phone/video.
Client is best supported through the assessment process, including assistance with completing forms/documents and/or attending assessments, where appropriate.
Full caseload is maintained by working within organisational guidelines.
Ongoing support and development by attending regular supervision, relevant training.
Ensure documentation meets organisational, legislative and funding requirements by completing and maintaining appropriate documentation and data in CRM.
Contribute to the continuous improvement of the program by assisting to refine policies, procedures and work practices.

Skills knowledge and experience:
Relevant tertiary qualifications in Community Services/Welfare/Aged Care/Social Work, and/or equivalent relevant qualification.
Previous experience in Home Care/Community/Aged Care or related services.
Exceptional interpersonal skills in communication, negotiation and problem-solving.
Current NSW driver’s licence and vehicle.
Intermediate computer skills in MS Office.
