About the Role
The Care Services Coordinator is a multifaceted role combining responsibilities of both Care Services scheduling, case management and administration supports for Seasons care clients. This role supports the Executive Manager Care and involves providing on site and remote support across the Seasons portfolio of communities as required to maintain business continuity and improve operational performance.
Core Responsibilities
- Provide support to Seasons Living communities to accurately schedule resident care and support services to meet client care needs as outlined within their care plan.
- Collaborate with Care Partners, Care Services Schedulers, and Care Managers to ensure services are scheduled effectively and efficiently.
- Support the teams to ensure the correct billing of all items including HCP, DVA, Private Fees, non-emergency buzzers, RN on Call, Day Respite, kilometers, and consumables for care clients.
- Manage unplanned services, ensuring all are correctly captured and billed accordingly.
- Review rostering and care plans, identify improvement opportunities, and provide guidance to minimizing unallocated time while considering labour costs.
- Consult with clients/representatives and a multidisciplinary team to assess client care needs, goals, and preferences to inform the development of holistic care plans.
- Maintain accurate and timely documentation covering assessments, actions, outcomes, risks, complications, and changes, meeting all medico-legal requirements and quality standards.
- Participate in an 'on-call' roster as required to ensure adequate staffing to meet client needs.
- Provide regional support to Executive Manager Care, assisting Seasons communities in monitoring and managing unplanned sick leave, and maintaining records of conversations and actions taken.
- Assist in maintaining accurate records within the quality management system (Centro Assist) and contribute to continuous improvement activities.
- Actively engage in quality improvement activities to enhance the quality of care and services delivered.
- Ensure compliance with all care systems, processes, and procedures, and participate in quality meetings facilitated by Seasons Care and Compliance Team.
- Develop and implement corrective action plans to address deficiencies identified through audits, observations, complaints, feedback, incidents, and surveys.

- 2yrs+ experience HCP management and/or clinical role managing teams and budgets.
- Demonstrated knowledge of the DVA Community Nursing Program, Home Care Packages, Aged Care Quality Standards and Client Rights and Responsibilities.
- Flexibility with hours when workload demands, across a 24/7, 365 day a year business.
- First Aid and CPR Certificate.
- Unrestricted Australian working rights.
- Current national criminal check.
