About the role
Catholic Homes is looking for a Case Manager to join our Home Care team on a full-time basis located in Burswood.
Our Home Care team builds lasting relationships with our Clients so that they may stay at home or get out into the community to do the things they enjoy.
Key Responsibilities
- Ensure a focus on quality improvement is maintained and nurtured in all areas of service delivery
- Ensure packaged services are provided within the limits of funding
- Organise daily schedule to meet consumer assessments and needs
- Demonstrate understanding of Aged Care Quality Standards, legislation, and partnership programs
- Provide on-the-job support and training to Support Workers as required
To be considered for this position, you will have/be:
- Minimum 2 years experience in case management in Home Care Packages
- Excellent understanding of Aged Care Quality Standards, client-directed care, and Community Aged Care
- Experience promoting Aged Care Home Care Packages
- Good understanding of the Wellness and Reablement model of care
- A roadworthy and registered vehicle and current C class drivers licence
- Excellent organisational, time management, and multitasking skills
- The ability to work well as part of a team and independently
- Well-developed communication skills including interpersonal skills
