About the role
As a Case Manager, you will be responsible for delivering case management to our clients that is person centred and enablement focused. You will be involved in all aspects of care coordination, including administrative tasks and rostering of client services. You will develop positive working relationships within the local service centre, and more broadly across the organisation to facilitate improved outcomes for our clients.
What you bring
Experience in community care case management
Experience developing care plans for complex clients
Tertiary qualifications in Community Services / Case Management or related disciplines
High level of computer systems proficiency, including data entry
Strong prioritisation and time management skills
A current and valid driver's licence
Willingness to travel across the Melbourne Metro region for Client visits
NDIS Workers Screening Check or willingness to obtain
Evidence of Covid-19 Vaccination
A desire to join a team committed to the care of others in a mission based organisation
