Geelong and Barwon Simply Helping is an NDIS and Aged Care Provider, dedicated to delivering exceptional in-Home Care. We are currently seeking a passionate and experienced Home Care Package (HCP) Case Manager to join our team.
Position Overview:
As a Home Care Package Case Manager, you will play a crucial role in coordinating and delivering high-quality care services to our consumers. The ideal candidate will have a strong background in care or case management within the Health, Aged Care, or Community Services Sectors, with a focus on Home Care Packages or the Community Home Support Program.
Responsibilities:
- Manage and coordinate Home Care Packages, ensuring the delivery of tailored care services to consumers.
- Conduct comprehensive assessments to determine consumers' care needs and develop personalised care plans.
- Collaborate with consumers, families, and multidisciplinary teams to ensure effective care coordination.
- Monitor and review care plans, adjusting as necessary to meet changing consumer needs.
- Provide support and guidance to consumers and their families, fostering a positive and empowering care experience.
- Ensure compliance with relevant standards, policies, and regulations.
What We’re Looking For:
- At least 2 years of experience in care or case management in the Health, Aged Care, or Community Services Sectors.
- Desirable: Nursing or Allied Health discipline with current AHPRA registration, or equivalent experience.
- Experience in case management or care management in either Home Care Packages or Community Home Support Program.
- Strong communication and interpersonal skills.
- Ability to assess and prioritise consumer needs.
- Knowledge of Aged Care standards and regulations.
- Excellent organisational and time-management skills.
- Empathy and a genuine commitment to improving the lives of individuals in need of care.
