Clinical Care Coordinator

Phoenix Source & Select has partnered with one of Tasmania's largest aged care provider. Not limited to residential aged care, retirement villages as well as home care; they employ over 1500 people across the state.

We are excited to share that we have multiple opportunities available for a permanent full-time Clinical Care Coordinator. We are seeking a Registered Nurse with Management experience, ideally within the aged care sector. We require the candidate to have an understanding of the Aged Care Quality Standards and legislative requirements. The roles are available at Launceston or George Town.

The role of the Clinical Care Coordinator is to provide clinical leadership, oversight, and advice in addition to supervision, and monitoring of RNs, ENs and ECA. The successful applicant will also provide operational leadership to deliver quality clinical care to consumers in accordance with operational and legislative requirements, including:

• Skills in leading and managing a clinical team
• Knowledge of continuous improvement for our systems and processes
• Capacity to achieve and deliver on regulatory compliance
• Collaborating with the health care team to achieve resident outcomes
• Participating in the development of a supportive environment with the team

Skills & Experience

Essential

• Minimum 12 months experience as a Registered Nurse in Aged Care or equivalent
• Bachelor of Nursing or equivalent
• Unrestricted AHPRA registration
• Sound clinical skills with a consumer focus
• Extensive knowledge of the Aged Care Quality Standards
• Knowledge and understanding of ACFI/AN-ACC processes, assessments, and reviews

Compliance requirements:

• Satisfactory national police check within the last six months
• Tasmanian Working with vulnerable persons registration with NDIS endorsement
• Tasmanian drivers’ licence
• Sufficient vaccination evidence or medical practitioner-approved exemption, as per current vaccination policy requirements, such as annual influenza and covid-19 vaccinations
• Satisfactory evidence of working rights in Australia
