Latrobe Regional Hospital are looking for a Registered or Enrolled Nurse to to join our Aboriginal Health Unit as a Clinical Care Coordinator on a part time ongoing basis. There are 48 hours available for this position, however we are open to job share arrangements for successful candidates wanting less hours.

In this position you'll coordinate integrated care for all patients of Aboriginal and/or Torres Strait Islander (ATSI) origin within our services and facilitate care within the community. You will be responsible for increasing patient and carer’s knowledge, confidence, and participation in their self-management, treatment and care planning, and will love the difference you can make everyday you come to work .

You may already have experience in a similar role or could be looking for the next step in your career – either way you’ll be well supported through our multi-disciplinary teams and a range of staff well-being activities.

Our ideal candidate will:
Be a Registered or Enrolled Nurse with AHPRA
Sound knowledge and understanding of Aboriginal culture, society and kinship networks as well as the ability to communicate with, and be accepted by, the Aboriginal community.
Previous experience with coordinating care for patients in the community
Be of Aboriginal or Torres Strait Islander decent is desirable (Please note: this is NOT mandatory and non ATSI applicants will be considered).
Well-developed interpersonal and communication skills, written and verbal, and the ability to develop and maintain communications with Aboriginal communities, mental health service staff, maternal staff and government agencies.
