Grampians Health Ballarat Diabetes service is seeking applications for a dynamic, energetic Clinical Nurse Consultant Diabetes Educator to join the current diabetes team on a fixed term part time basis. As a valued member of the team, the sucessful candidate will deliver evidenced based person centred care across a range of settings. These include acute admissions and outpatient specialist clinics. Opportunities for collaboration, professional development and networking all contribute to professional growth and will support credentialling requirements with Australian Diabetes Educators Association (ADEA).
The Clinical Nurse Consultant Diabetes Educator provides specialist services including educational services and therapeutic direct clinical care. The team approcat to care is in collaboration with the medical workforce, nursing staff, allied health, pharmacy and consumers to deliver person centred care to service users across the lifespan.

- Essential : Do you hold current registration with the Australian Health Practitioner Regulation Agency (AHPRA)? Please detail your registration number.
- Essential : Do you hold a post graduate qualification in Diabetes Education? Please attach this with your application.
- Essential : Have you received three doses of a COVID19 vaccine? If no, please provide further detail regarding doses received, booking dates and/or other relevant information.
- Essential : The successful candidate must hold, or be prepared to undergo, an employee Working with Children Check (WWCC). Please advise if you currently hold an employee WWCC, if not are you willing to undergo one?
