Clinical Nurse Consultant - Older Persons Team

About the role:
This community-based position will require you to provide intervention and consultation to older individuals in the home, community health setting and Residential Aged Care Facilities.
The Older Persons Mental Health Services are a clinical stream of public mental health services generally provided to people aged 65 years and over, and 50 years and over for Aboriginal people. Older Persons Mental Health Services include: multidisciplinary community, outreaching into hospital wards and community Older Persons Mental Health Services partnerships, including residential services and programs.
Provide expert clinical consultancy in Infection Prevention to support the delivery of excellent patient/client care within the HNELHD Mental Health Services.
Staff will be provided with education and support from a supportive Multi-Disciplinary Team which consists of Service Manager, Team Manager, Clinical Coordinator, Clinical Nurse Consultant at Forster and Taree.
Clinical supervision is offered to all clinical staff members, The service also provides a super nummary period to allow staff to adjust to Community Mental Health.
The position will be supported with frequent one to one with managers to drive and deliver excellence in our Mental Health Service.
Staff are enabled to access workforce development opportunities that support life-long learning in accordance with relevant NSW Health policies and further develop the specialised skills, competencies and knowledge required to work in the older person mental health setting.
About you:
The successful candidate will have demonstrated knowledge and experience within a mental health setting and the ability to take charge and case manage clients within Taree & Forster and surrounding areas.
You will also have the ability to assess risk and perform mental state examinations and your exceptional communication skills and passion for building therapeutic relationships will be the key to your success in this position. Deliver consumer centred services that engenders trust and confidence is about older people with, or at risk of, mental illness, their needs, and the needs of those who care for and support them.
Information for Applicants:
All NSW Health workers are required to have completed a primary course of a COVID-19 vaccine which has been approved or recognised by the Therapeutics Goods Administration (TGA). Additionally, Category A workers are required to receive a booster dose three months after completing the primary course of COVID-19 vaccinations. New applicants must have completed the vaccination course prior to commencement with NSW Health, or provide an approved medical contraindication certificate (IM011 immunisation medical exemption form) certifying the worker cannot have any approved COVID-19 vaccines available in NSW.
Acceptable proof of vaccination is the Australian Immunisation Register (AIR) Immunisation History Statement or AIR COVID-19 Digital Certificate. Booster doses are highly recommended for all health care workers who have completed the primary course of COVID-19 vaccinations.
