About the service
Planned Care for Better Health and Emergency to Community, is co-located on the Adamstown site with the Community Referral Intake Centre and Integrated Chronic Care for Aboriginal People Program. The Services are supported by a Nurse Manager and include administration and nursing staff. The service operates Monday to Friday, 8.00am - 4.30pm, and is closed on public holidays.
About the role
The successful candidate will provide leadership to support best practice clinical and non-clinical care for vulnerable individuals referred to the Planned Care for Better Health service. You will provide expert direction within the team, to achieve the goal of improving health and social outcomes for individuals via a combination of extensive, individualised care coordination, care navigation and health coaching.
You will be supported by a Nurse Manager and the broader team, including administration and nursing staff. A comprehensive orientation program is provided, with access to training for job specific requirements.

The PCBH / ETC Team, are looking for an enthusiastic team player who loves a challenge and the chance to be involved in an innovative service which is rapidly changing to meet the needs of the most vulnerable people in our communities. This role provides the opportunity to learn new skills, apply your well-developed time management, organisational, advanced clinical and communication skills, to work with passionate clinicians in the provision of patient focused care via a predominantly telehealth platform. You will proactively contribute to the development and evaluation of creative care strategies to achieve tailored clinical and non-clinical outcomes for each individual.

All NSW Health workers are required to have completed a primary course of a COVID-19 vaccine which has been approved or recognised by the Therapeutics Goods Administration (TGA). Additionally, Category A workers are required to receive a booster dose three months after completing the primary course of COVID-19 vaccinations. New applicants must have completed the vaccination course prior to commencement with NSW Health, or provide an approved medical contraindication certificate (IM011 immunisation medical exemption form) certifying the worker cannot have any approved COVID-19 vaccines available in NSW.
Acceptable proof of vaccination is the Australian Immunisation Register (AIR) Immunisation History Statement or AIR COVID-19 Digital Certificate. Booster doses are highly recommended for all health care workers who have completed the primary course of COVID-19 vaccinations.
Occupational Assessment, Screening and Vaccination against Specific Diseases - this is a Category A position. Please read and understand NSW Health policy directive (PD2022_030). All new employees must agree to comply with the requirements outlined in the policy.
To be eligible for permanent employment in this position you must be an Australian Citizen, or a permanent resident of Australia, or a New Zealand citizen with a current New Zealand Passport.
