Being able to keep or regain independence and enjoy life from the comfort of home and a safe environment is of great importance to our clients. As a Community Connection Support Officer you will support clients to connect with informal and formal supports to increase their independence and reduce social isolation. 
  
About the roles

Reporting to the Service Leader, the Community Connection Support Officers will work with clients to explore services to increase their level of independence and overall wellbeing in preparation for discharge from the QCSS program. This may include seeking employment services, medical supports, housing, education providers, culturally specific supports, volunteer roles and social clubs. These roles will support clients living across Brisbane, and stretching out to Fraser Coast, Sunshine Coast, Moreton Bay, down to the Gold Coast and out to Ipswich and the Scenic Rim.

About Yourself
  • Have a genuine interest in supporting clients to live independently within their own community
  • Have excellent communication and time management skills
  • Display a cheerful outlook, are consistently reliable, honest always, adhere to safe work practices and understand and work within professional boundaries
What you’ll need 
  • Experience in Disability, Mental Health and Community Sector - preferred
  • Certificate IV in Community Services, Disability Services, Mental Health services – preferred
  • Strong understanding of the Queensland Community Support Scheme legislation requirements along with Human Services Quality Framework standards
  • Strong organisational and time management skills
  • Excellent communication and relationship-building abilities
  • Knowledge of compliance processes and legislative requirements
  • Excellent communication skills and experience
  • Be able to obtain/ have a Blue Card, NDIS Worker Screening Clearance 
  • Driver’s licence
  • Police Check
