What You Will Be Doing
The Community Care Physiotherapist forms part of Peninsula Health's Health Independence Program and is based on the Wagner Chronic Care Model, an evidenced based systems framework.
The Community Care PT is responsible for coordinating intensive short term (3-6 months) outreach care and delivering allied health interventions within their clinical scope of practice to clients who have chronic health conditions and/or complex healthcare needs.
What You Need
We anticipate you will have:
- Current registration with Australian Health Practitioner Regulation Agency (AHPRA) or relevant professional organisation.
- Demonstrated Occupational Therapy or Physiotherapy experience in providing comprehensive discipline specific clinical assessment in home setting and provision of equipment needs.
- Specialist experience and knowledge regarding chronic disease management.
- Demonstrated experience working with at-risk and vulnerable people with complex needs.
- Current Victorian Driver's Licence.
- Demonstrated experience in working both independently and as a part of a multi-disciplinary team, with the ability to work effectively, supportively and flexibly.
- Demonstrated ability to manage multiple competing tasks and effectively prioritise.
- Demonstrated commitment to professional development and evidence based practice.
- Demonstrated ability to supervise students.

Selection Criteria Essential: Current Physiotherapist registration with Australian Health Practitioner Regulation Agency (AHPRA)
