Job description

The Community ConnectorAged Care will support identified clients to navigate access to appropriate Aged Care support services. They will provide advocacy support to ensure clients receive a culturally safe plan assessment. They will connect and engage older people who have significant difficulty accessing Aged Care services and are at risk of “falling through the cracks.”

This role will support clients to understand their Aged Care service entitlements, including My Aged Care registration, contractual arrangements, and costs, and will help ensure Elders and their families are well-prepared for plan reviews and outcomes. This role will liaise with Aged Care assessors to ensure Elders are well-supported throughout the assessment process.

This role is underpinned by two key work principles: cultural safety and trauma-informed practice.

Specific requirements of this position include but are not limited to;

Essential Criteria

  • Experience in the community Aged Care sector with demonstrated experience in assessment, care coordination/case management or advocacy, or other relevant role within a community services context.
  • A demonstrated understanding of diversity issues as they relate to individuals and their engagement with community care services
  • Excellent interpersonal skills.
  • Experience in planning and delivering community events.
  • Demonstrated understanding of the principles of person-centered care and trauma-informed practice.
  • Highly developed verbal and written communication skills with a demonstrated ability to write in a way that is clear, appropriate, and readily understood by the target audience
  • A demonstrated understanding of the unique issues affecting and impacting the health of Aboriginal people

Desirable Criteria

  • Aboriginal or Torres Strait Islander people are encouraged to apply.
  • Familiarity with Aged Care services and the local Aged Care system including referral pathways, services, and advocacy
  • Demonstrated ability in implementing, monitoring, and evaluating accreditation standards
  • Ability to understand, interpret, and report on client data related to client health outcomes
