About the role
The Community Care Unit (CCU) service provides intensive treatment and recovery support in a residential setting of up to two years to help people stabilise their mental health and gain the necessary skills to transition back into the community from a hospital environment. The CCU service delivers 1:1 peer support, group peer-led activities and support services that align with individual’s choices and aspirations identified in the recovery plan. Clients are supported to define their recovery goals and health outcomes, build capacity and living skills, strengthen social and community connections, develop support networks and grow confidence to live independently.
We are seeking a resilient, compassionate Community Mental Health Practitioner - Aboriginal & Torres Strait Islander for a 24 month fixed term, part time (30 hours per week) opportunity to work on a 24/7 rotating roster on Gunggandji Country. Working with Aboriginal and Torres Strait Islander peoples, you will support implementation of the RAP and provide clients with person-centred care to achieve their recovery goals, build capacity and life skills, enhance connections with the local community and elders and work towards independent living.
Key responsibilities
- Support clients by delivering high quality services and facilitating positive educational and social experiences.
- Build trusted relationships through strengths focused engagement with local communities, kinship groups and elders.
- Provide consultation to the team and stakeholders to raise awareness of health outcomes for Aboriginal and Torres Strait Islander peoples.
- Assist clients to reconnect or connect with local Aboriginal and Torres Strait Islander communities and services.
- Collaborate with a range of services and the community to ensure a co-ordinated, integrated response to the client’s recovery goals.
- No minimum qualification required but a Tertiary qualification (minimum Certificate IV) in Mental Health, Psychology, Social Work, Occupational Therapy or other health related field is desirable.
- Able to provide a record of Vaccination Preventable Diseases or able to obtain vaccinations against Measles, Mumps, Rubella, Chicken Pox, Whooping Cough, Hepatitis B or other diseases as required by our Partnership with QLD Health.
- Expertise in working with Aboriginal and Torres Strait Islander peoples with mental health issues who have complex needs and strong experience interacting with communities, families and carers.
- Understanding of the link between Aboriginal and Torres Strait Islander health outcomes, homelessness and mental health.
- Proven capability to plan goal focused care and implement therapeutic, recovery-oriented frameworks and practices.
