Position Summary:
A vacancy exists in the above unit for a highly motivated Social Worker with appropriate knowledge and skills to join our friendly and supportive team. Our progressive and innovative team offers development opportunities for our staff and a caring environment for the delivery of high quality patient centred care. The successful applicant(s) will be expected to be able to work in our Continuing Care Teams as a case manager for people with mental illness.

- Current registration with Australian Association of Social Workers
- Demonstrated commitment to ongoing professional Development
- Demonstrated ability to work as part of a Multidisciplinary team with excellent communication and interpersonal skills and Demonstrated understanding of recovery principles and commitment to a collaborative, strengths based approaches to the care of mental health consumers.
- Proven experience in the provision of care and assessment, and management of risk in community setting.
- Demonstrated effective time management skills and ability to priorities competing workloads with in a complex environment
- Demonstrated computer literacy and proven ability to use health information systems.
- Evidence of a current unrestricted drivers’ license and the ability to maintain.
