COMMUNITY NURSING ASSESSOR (RN DIV 1)-HOME CARE PACKAGES

mecwacare is looking for an experienced and motivated Registered Nurse to join our Home Care Packages Team. This role is instrumental in providing support and advice to the Home Care Packages clients to assist them to continue living at home. The Clinical Nursing Assessor is primarily responsible for receiving referrals and undertaking holistic clinical assessments and reviews for clients within the Home Care Packages (HCP) team using the approved tools and documentation.
 
What you bring:

  • Tertiary qualifications as a registered nurse Division 1 and current registration with AHPRA.
  • Experience in undertaking holistic assessments and care planning of clients within a community setting
  • Excellent verbal and written communication skills.
  • Ability to establish rapport and engage clients from diverse backgrounds.
  • A comprehensive understanding of the physical and psychosocial issues experienced by individuals who require assistance in daily living and their carers.
  • Strong IT skills including Microsoft Office suite.
  • Ability to work autonomously including being well organized and possessing excellent time management skills
  • Previous experience within an aged care environment and familiarity with the community services network.
  • Knowledge of Occupational Health and Safety principles and systems.
  • Current Victorian Driver’s Licence.
