About the Role

This position has two components:

Complex Care Coordination: you will provide person-centred care coordination for clients living with complex psycho-social and healthcare needs, utilising discipline specific skills to support clients to improve their self-management skills. As a Social worker or Occupational Therapist you will conduct discipline specific assessments and provide appropriate discipline specific clinical management, discharge planning & community liaison for all relevant patients.

Direct clinical provision of high quality and evidence-based patient centred care.

Demonstrated advanced knowledge, skills, abilities and attributes, including critical thinking and reasoning, advanced problem-solving skills and expert clinical judgement

Provide advanced discipline specific assessment and intervention for clients and secondary consultations to staff across HIP sites.

Identify and access multidisciplinary and specialist supports to address specific clients issues

Undertake holistic comprehensive assessments in collaboration with the client using motivational interviewing and self-management approaches.

Collaborative care planning with clients, carers and/or families in the context of their health, functional, psychosocial, cultural, environmental and economic needs.

Facilitation of client generated goals that are realistic, time limited and based on addressing health and wellbeing needs.

​Classifications for this position will range from:
Social Worker Gr 2 SC21 - SC24 ($83,361 - 95,825 per annum)

Occupational Therapist VF6 - VF9 ($83,361 - 95,825 per annum)

Your Contribution

Allied Health Professional Registration with AHPRA (where required)

Minimum 3 years’ experience in relevant acute/community setting

Current Victorian Driver’s licence

Experience in working with aged clients living with chronic diseases and complex health and psychosocial conditions.

Ability to perform comprehensive assessments and develop collaborative plans that address physical, psychosocial and functional aspects of the client.

Skills and expertise in specific clinical area such as Gerontology, Mental health and substance use recovery.

Ability to make sound decisions based on risk assessment and develop action plans to address issues for vulnerable clients.

Current National criminal history check, or willing to obtain

Health Independence Program

This role is part of the Health Independence Program (HIP). The intent of HIP is to deliver improved outcomes for clients and to support hospital demand management and flow, through delivering integrated and coordinated care to clients across the hospital and community interface, reducing duplication and fragmentation of services and ensuring client centred care planning.

This position will be part of the HIP Complex Care Team (CCS). The key aim of this team is to improve health outcomes to people who either frequently presents, or who are at risk of presenting to hospital, include patients with complex needs who fall under one or more of the key risk factors; substance use, mental health issues, homelessness, disability and the complex aged, family violence
