This is a Fixed Term Full Time appointment until 30/10/2024 at 76 hours per fortnight, with the possibility of extension and/or permanency.

Whilst this selection process will initially be used to fill the above vacancy, it may also be used to fill other "similar" vacancies throughout the health service. In addition to this, should the successful applicant decline or vacate the advertised vacancy, then the next most suitable applicant may also be selected from this process. Both of these options remain valid for a period of twelve (12) months from when the authorised delegate endorses the recruitment decision.

Position Profile: To support children and young people in out of home care navigate the WA Health system, by coordinating health management planning in a culturally appropriate environment. The successful applicant will liaise with children, young people, their carers, families, child protection workers, community organisations and agencies to build health profiles and manage ongoing health planning. The position will be responsible for the facilitation of health assessments, improving health literacy and streamlining health processes for children in out of homecare. It will include coordination, implementation and evaluation of the Health Navigator program. Facilitate the ongoing strategic partnerships with key stakeholders and provide expert advice related to service changes and program redesign.
