About the role:
- Rural Rehab is a Cardiopulmonary outpatient rehab service that offers a time limited intervention with a multi-disciplinary team.
- Provide a high quality clinical service to clients/patients/consumers and their families/carers to ensure the provision of optimal outcomes for clients/patients/consumers of the service with a Chronic Disease.
- Evidence based and practice based interventions that encourage personal responsibility and self-management in the process of recovery.
- You will be supported by clinicians providing service and Nurse Unit Manager.
- You will also be is supported by the HVC Dietitian Head of Department and Dietitian support group.
- You will be working within a multi-disciplinary team consisting of a Registered Nurse, Physiotherapist, Social Worker and Dietitian.
- The Dietitian service is provided across the 6 Rural Rehab sites (Scone, Muswellbrook, Singleton, Cessnock, Kurri Kurri and Maitland).
- Service provided out of existing Hospital Physio Gyms.
- Must hold a bachelor or post graduate degree in nutrition and dietetics that provides eligibility for full membership of the Dietitians Association of Australia, or other qualification deemed equivalent by the employer.
- Eligibility to drive in NSW.
- Capable in using CHIME or willingness to complete the necessary training.
- Four weeks annual leave (pro-rata for part-time employees)
- Superannuation contributions 11%
