As our Central Qld office continues to grow and we are looking for an experienced and dedicated Enrolled Nurse/Care Coordinator to support this continued growth in the Central Qld Region.
The successful candidate will preferably have a strong background in Community Aged/Disability Care. The role itself is responsible for service development, coordination and delivery, including ongoing client assessments and development of care plans that deliver flexible and personalised in-home support. Direct nursing support to clients is also incorporated within the role, as such a current AHPRA-Nurse Registration is required (EN or RN). Successful applicants will thrive on working in a close-knit team and have a passion for delivering the highest quality in client care.
This role does require some flexibility as some weekend supports are likely to be required, with this time to be offset as reasonable through the week.
This key role will focus on case management, nursing support, customer service, personal care work where required and general administration.
Major duties include:
Care Assessments;
Government-Compliant Care Management Plans;
Holistic outcomes driven Care Planning;
Operation of the Care Planning and other management systems;
Nursing and personal care supports; and
Quality assurance.

Qualifications
Qualifications;
Minimum of Diploma in Nursing with a current AHPRA registration;
Current First Aid & CPR
Driver's License
Blue Card/Yellow Card
Police Check
Vehicle Registration and Insurance
Essential Criteria
Must have a Sound understanding of the principles of My Aged Care, Aged Care Reforms, Consumer Directed Care
Must have Significant experience and understanding in the Australian Aged Care or Disability sector
Must be Empathetic, passionate and caring
Must have Experienced in assessing and planning care needs
A client focus which is based on their goals, preferences and needs
Must Demonstrate experience managing staff
Must have a Comprehensive Assessment and Case Management experience
Must have Experience in provision of care and services to people from culturally and linguistically diverse backgrounds
Must have a Commitment to work within RAH Mission, Vision and Values
Must have the Capacity to take initiative and to be innovative
Must have a Highly developed organisational, administrative, business, people, and financial management skills with proven ability to determine priorities, coordinate activities and meet deadlines
Must have Customer service experience (in the Aged Sector essential)

Good understanding of the Government funding environment for aged care & NDIS (preferred but not necessary);
Good understanding of the hospital discharge environment;

It is also essential to have good judgment, quick decision-making, excellent communication, staff management and administrative skills; and
