Grade 2 Allied Health Clinician – Community Navigator

The Community Navigator role works with existing teams to provide individualised patient centred discharge planning for COVID positive patients and within allocated medical units. Patients are diverse and often have complex needs, and therefore there is a strong focus on ensuring services are integrated on discharge home or to other settings to improve inpatient flow and prevent readmissions. This is a dynamic position to ensure this 7-day service is delivered to its full potential.
Your new role:
- Provide clinical evidence based best practice and deliver patient-centred care across the 7-day service.
- Work autonomously and within the team to provide early discharge planning and coordination of medical patients.
- Develop relationships with a diverse group of internal and external stakeholders.
- Provide COVID Community Navigator services as indicated in response to hospital needs.
- You are an Allied Health professional with clinical expertise in patient management and have worked in a hospital environment.
- You have a high level of clinical reasoning and clinical knowledge.
- You have well developed communication skills that allow you to build relationships with a wide variety of stakeholders.
- You are highly organised and able to effectively manage competing priorities.

All appointments are made subject to a satisfactory Criminal History Record Check and, where applicable, Working With Children Check (WWCC).
