Emerge Australia is the national patient organisation providing education, advocacy, research and support services for people living with myalgic encephalomyelitis/chronic fatigue syndrome (ME/CFS). Our work gives hope and help to more than 250,000 Australians living with ME/CFS. We:
- deliver patient-centered, consultative projects to empower patients and encourage self-management;
- increase patient literacy with well-informed, medically sound information, aligned with global consensus; and
- have strong sector connections with the ME/CFS community, health professionals, government and leading ME/CFS research groups across Australia and internationally.
Role overview
The Health and Social Support Liaison Offer will be at the heart of our patient support service. You will support the empowerment of people who live with ME/CFS and Long COVID, their carers and support service providers by providing navigation and linkage for individuals seeking support.
You will act as a bridge between individuals who live with or care for a person with ME/CFS or Long COVID and in need of health literacy, support services and understanding of various resources available to them. In the first instance, you will link people to Emerge Australia’s education, patient-specific services, or information packages. Secondly, the Health and Social Support Liaison Officer will help individuals to link in with external services by either direct referral where possible or by helping the individual to reach out for support. The roles will be supported by the Emerge Australia professional team who can provide additional help for extremely complex and difficult cases.
Our team at Emerge Australia work cross-functionally and collaborate to achieve optimal outcomes for our community and you will support our team by linking the community in with all projects and advents.
Key selection criteria
- At least a minimum of 3 years' experience in a support or linkage role.
- Ongoing professional development.
- Experience in talking to people with complex social situations.
- Evidence of talking to people with chronic disease.
- Some understanding of Australia’s Health and disability systems
- Ability to and willingness to learn about the experience of people who live with ME/CFS and Long COVID
- Ability to maintain appropriate records, statistics and generate relevant reports
- Excellent computer skills and the ability to use Microsoft products and CRM
