POSITION OVERVIEW
The Transitional Therapy and Care Program (TTCP) provides up to 12 weeks of goal focused therapy and care services for older persons within their home or at the TTCP residential unit. The TTCP operates from the University of Canberra Hospital, Bruce. The TTCP aims to maximise a person’s recovery and functional independence following a hospitalisation and prevent premature admission to an aged care facility. The TTCP Allied Health team is Multidisciplinary and consists of Physiotherapists, Occupational Therapists, Allied Health Assistants, Dietitians, a Social Worker, and a Speech Pathologist.
The TTCP Liaison Officer is responsible for coordinating a patient’s transition from hospital onto the TTCP. This is achieved through liaising with the patient, families and/or carers and ward staff to coordinate appropriate referrals, assessing clients for suitability, discussing the program with all key stakeholders, referring to the Aged Care Assessment Team (ACAT) for assessment and approval for TTCP and facilitating discharge from hospital onto the program. The TTCP Liaison Officer is required to work at multiple sites including public and private hospitals within the ACT.
This position will also include a clinical role at a HP3 level within the position holder’s profession. The TTCP Liaison Officer will manage their clinical case load in conjunction with the requirements of the Liaison Officer position.
This position is a temporary full-time position with potential for permanency. A merit list will be established from this recruitment to fill future temporary and permanent positions in a full-time or part-time capacity.
DUTIES
Under limited direction of the TTCP Manager you will be responsible for the provision of high quality Liaison Officer services, clinical assessments and interventions to the ACT community. Promoting positive client outcomes through the provision of high quality clinical services, networking, health promotion activities and education in/across designated areas or units as part of a multidisciplinary team. You will:
- Provide a high level of appropriate skilled clinical assessment regarding patient suitability for admission to the TTCP
- Liaise with patients, carers, ward team, the ACAT, the TTCP Allied Health team and TTCP service provider to facilitate effective admission to the TTCP following discharge from acute or sub- acute hospitals
- Carry a clinical workload within your profession and remain current in specialised knowledge, expertise and skills through continuing professional development and ongoing education.
- Contribute to the planning, development and evaluation of client programs and services, including the evaluation and analysis of policies and procedures applicable to professional practice and coordination of quality improvement activities in areas of speciality.
- Provide professional supervision and training to Health Professionals Level 1 & 2, students, AHAs and other support staff whilst also participating in your own professional education, supervision and personal performance review process.
- Consult and liaise with external community agencies/groups to develop partnerships for health promotion and other service delivery activities identified in the business plan.
- Undertake other duties appropriate to this level of classification which contribute to the operation of the organisation.

Position Requirements/Qualifications:
Mandatory
- Relevant qualifications and a minimum of 3 years’ experience working professionally in a relevant clinical area
- Be registered or be eligible for registration with the Australian Health Practitioner Regulation Agency (AHPRA) or relevant Allied health Professional Organisation.
- Current Driver’s License.

Computer literacy skills are required which are relevant to this role as you will be responsible for completing required documentation and becoming a proficient user of the Digital Health Record and/or other Information Technology systems; once proficient, you will need to remain current with changes, updates and contingencies.

Prior to commencement successful candidates will be required to:
- Undergo a pre-employment National Police Check.
- Obtain a Compliance Certificate from OMU (Occupational Medicine Unit) relating to assessment, screening, and vaccination processes against specified infectious diseases.

- Comply with CHS credentialing and scope of clinical practice requirements for allied health professionals.
WHAT YOU REQUIRE
These are the key selection criteria for how you will be assessed in conjunction with your resumé and experience:
- Proven knowledge of and ability to provide a high level of skilled clinical assessment for potential referrals for the TTCP. Extensive experience in assessing, planning and implementing therapeutic treatment in your clinical area for clients with neurological, orthopaedic, aged, general medical and/or surgical conditions in a rehabilitation or aged care setting.
- High-level interpersonal and communication skills, including demonstrated ability and experience to liaise, negotiate and communicate with a range of staff, patients, carers, agencies, service providers and in the supervision of Health Professionals in your specific profession, Allied Health Assistants and students.
- Proven ability to work effectively both autonomously and within multi-disciplinary teams, contributing positively to team operations, working relationships and positive patient outcomes through excellent communication skills and developing leadership skills.
- Proven commitment to the delivery of patient centred services that meet high safety and quality health care standards with evidence of quality improvement activities and/or research and knowledge of current policies and trends within the context of a rehabilitation and aged care setting.
