A vacancy exists in the Social Work Department for a highly motivated Heart Transplant Senior Social Worker and Team Leader Level 4 with appropriate knowledge and skills to join our friendly and supportive team. Our progressive and innovative team offers development opportunities for our staff and a caring environment for the delivery of high quality patient centred care. The successful applicant will be expected to be able to be responsible for the management and leadership of a social work team providing high level service to patients, clients, their families and carers for a state-wide service including heart and lung transplants for patients from within NSW and interstate. This senior role shall provide a consultancy service in their area of expertise, particularly transplantation and organ donation, for Social Work, for other health care professionals within St Vincent’s Hospital and with external agencies.
The Heart Transplant Senior Social Worker Team Leader will be required to ensure clinical service provision and facilitate comprehensive social work service throughout the Heart Lung Transplant Clinical stream comprising ICU, inpatient and outpatient services.
This senior role shall provide a consultative service for social workers, all St Vincent’s health professionals and external agencies responding to crisis health needs across the St Vincent’s Health Network. The level 4 Senior Social Worker is required to represent the Social Work Manager, Social Work Department and St Vincent’s at relevant committees, meetings, forums and participate in the development of related policies, procedures and services within the Social Work Department and across the community and Network. This Senior Social Work position is involved in the planning, implementation, evaluation and reporting on relevant social work services across the St Vincent’s Health Network as required by the Manager of Social Work. Additionally, this position is required to provide advice to the Social Work Manager on clinical service delivery, development, practice and redesign. This senior social worker shall be expected to provide supervision to staff and students, conduct education sessions for staff of all disciplines, provide training and support excellence in clinical practice founded in current evidence. Other specialist duties may be assigned as required by the Social Work Manager or the Director of Allied Health.
The role will work predominantly Monday to Friday with availability to work on call as part of a seven day service.

About you:
We are seeking a skilled and energetic Senior Social Worker level 4 who possess a passion for person centred care. We welcome applications from people who are highly skilled, experienced and motivated for the provision of social work team leadership and delivery of social work services across the Heart Lung Stream at SVHS. This is in addition to comprehensively managing a complex personal caseload of inpatient and outpatient Heart and Lung Transplant patients.

Selection Criteria
- Personal integrity and demonstrated commitment to the Philosophy, Mission and Values of Mary Aikenhead Ministries and St Vincent’s Health Australia.
- Bachelor of Social Work or equivalent tertiary qualification with eligibility for membership of the Australian Association of Social Workers (AASW).
- Demonstrated expertise, legislative knowledge, skills and experience in Tertiary Health in critical illness as a senior social worker in: Intensive Care, transplantation, organ donation, Heart and Lung with high level expertise in risk assessments for: domestic violence, child protection, postpartum, guardianship, Legal/Visa status, sudden death, mental health, drug and alcohol.
- Demonstrated competencies, specialist knowledge and advanced practice skills in crisis intervention, trauma informed counselling, adjustment to illness and disability, short term solution focused intervention, grief and loss, complex assessment including capacity assessment, care and discharge planning.
- Demonstrated commitment to and high level experience in multidisciplinary team work in a high demand environment with competing priorities.
- Demonstrated team leader experience in managing a team of social work professionals and in the provision of clinical supervision and education of staff.
- Experience and high level capability to lead clinical planning, research, quality improvement and education projects within social work services and the Heart and Lung Stream for SVHN
- High level oral, written, negotiation and computer communication skills including tele-health with demonstrated experience in providing a consultancy service in Health to service managers.
