Who are we?

Wintringham provides in home aged care services to those who are financially disadvantaged and are homeless or are at risk of homelessness. We employ over 900 staff and operate from multiple offices across Melbourne, Hobart and regional Victoria. Wintringham is a major provider of in home care and support services to this vulnerable client group. Our in house team of Care and Support Workers, Case Managers, Outreach Workers and Recreation staff provide holistic care and support to clients who live independently in their own homes. The care provided to Wintringham clients is carried out with the underlying principles of social justice.


Your new role

Wintringham is seeking motivated and enthusiastic Case Managers with a commitment to social justice to join our multi-disciplinary community teams based at our offices across metropolitan Melbourne and regional Victoria on a casual basis. We are looking for Case Managers who are flexible in their availability as well as having the ability to cover unplanned leave (i.e. sick leave) and periods of planned leave. We are also open to employing new graduates and final year students of relevant disciplines who may be looking for experience in this role. Excellent supervision is offered all staff.


Your duties and responsibilities will include:

  • Assisting clients living in their own homes to maintain their independence and lifestyle of their choice by providing a high level of care
  • Helping address issues of social isolation for program clients and assist clients to maintain or improve their accommodation
  • Undertaking initial assessments for new clients and ensure ongoing reassessment at any time that the client’s care needs change
  • Developing and implementing care plans
  • Maintain accurate and timely records of client’s assessment, care plan and services provided
  • Participate in care reviews with managers, clients and carers
  • Effectively managing services provided within budgetary constraints
  • Working with an in house rostering team to ensure clients receive the care and supports they require according to their funded level of package

We are looking for candidates with the following:

  • An approved tertiary qualification in Social Work, Nursing, Allied Health, Nursing or Welfare (or related discipline) OR in final year of Social Work or other Allied Health courses.
  • Previous experience as a Case Manager (would be highly regarded)
  • High level computer, documentation and budgeting skills (mandatory)
  • Current drivers licence with a reliable vehicle suitable for transporting clients
  • Knowledge of housing and support issues facing older homeless people whom may have a physical and/or mental disability.
  • Skills in assessing clients’ psychosocial, care, support, health and housing needs.
  • Availability – minimum 3 business days per week, flexibility around working from home. Work hours may vary depending on site leave
