We are looking for two dedicated Home Care Coordinators to join our friendly Home Care Services team at our central office in Glenside; supporting our Southern and Eastern regions respectively.
About the role
The Home Care Coordinator will provide direct administrative and clinical care support services under the direction of the Wellbeing Partner (Coordinator) to deliver high quality, agile, positive and responsive Home Care.
In this role you will assist with client onboarding, assessment, escalations, purchasing, upgrades and reviews as well as liaising with key stakeholders including rostering, finance/admin teams and other home care or Southern Cross Care employees.
You'll work amongst a multi-disciplinary team to provide integrated care that promotes healthy ageing and wellbeing, social engagement, prevents frailty and avoidable decline and achieves reablement for clients to optimise their ability to live better in their own home.

About you

You’ll have the ability to demonstrate integrity, calmness and personal resilience. Passionate about person-centred care, enabling clients to maintain maximum independence, you will be able to seek creative solutions in your approaches to overcome any barriers.
With your excellent customer service, communication and time management skills, your bright and friendly disposition and exceptional telephone manner, you will establish positive working relationships with all staff and stakeholders.
You will require knowledge of the Google Suite and Microsoft Office applications. You will also be required to operate in various internal software systems.
