Opportunities are now available for General Practitioner Visiting Medical Officers (GP VMO) wishing to work for ISLHD’s Virtually enhanced Community Care (VeCC) which provides expert medical assessment and management of patients through ISLHD’s Virtual Care Centre, with respect to specified chronic diseases, initially chronic respiratory diseases and cardiac failure with potential expansion to a limited number of other acute-on-chronic or chronic conditions.

About VeCC

Virtually enhanced Community Care (VeCC) is a multidisciplinary service that supports patients to better self-manage their health conditions in the community, improving patient health outcomes whilst prioritising patient safety and reducing potentially preventable hospital admissions. VeCC provides both face-to-face and virtual care in the community including remote monitoring, care navigation, care coordination and health coaching interventions.

Specialty support is provided by ISLHD Physicians in the relevant subspecialty

Our vision for VeCC 2023 and beyond is to expand the scope of work to also include patient management of Paediatric and Surgical patients virtually, in conjunction with specialist input respectively.

About the role

This Virtual Care role requires GPs to provide virtual medical support to patients within the local community, particularly those with Covid.19, chronic respiratory disease and/or cardiac failure. Direct referrals are also provided by Illawarra GP’s to avoid hospital admissions. The current model is designated as a virtual hospital

Responsibilities include but are not limited to reviewing patients with any clinical signs or symptoms of concern as escalated by nursing staff, arranging investigations, developing management plans and referral of patients where necessary.

Patients of VeCC are provided with access to remote monitoring of their blood pressure, oxygen levels, temperature, blood glucose levels, weight, and symptoms allowing real-time assessment by clinicians

These assessments are within the routine scope of practice for GPs.

Roster requirements: VeCC operates 7 days per week, from 8am to 8pm. A commitment to set shifts of a minimum 6 sessional hours per day for patient reviews is essential. Preference given to those able to work 2 to 3 and up to 5 consecutive days per week. Patients are referred to ED if issues occur between 8pm and 8am the following day.

Where you will be working

Working location is currently in a location of the Medical Officer's choosing, access to systems will be enabled by internal Virtual Private Network (VPN). There is possibility for medical, nursing and allied health services to operate from a central location such as Piccadilly Community Health for some face to face reviews in the future, depending on the condition being managed and the patient’s state of health.

The Illawarra Shoalhaven Local Health District (ISLHD) has responsibility for health services in the Illawarra Shoalhaven region – a catchment area extending along the coastline from Helensburgh in the north to North Durras in the south, with over 400,000 residents. This catchment covers four Local Government Areas (LGAs): Wollongong, Kiama, Shellharbour and Shoalhaven. We operate eight hospital sites: Coledale, Bulli, Wollongong, Port Kembla, Shellharbour, Berry, Shoalhaven and Milton Ulladulla, and community health services operating from approximately 58 locations across the region. ISLHD is one of the region’s largest employers with a workforce of more than 8,500 staff.

All NSW Health workers are required to have completed an up-to-date course of a COVID-19 vaccine (3 doses for Category A roles, 2 doses for Category B roles) which has been approved or recognised by the Therapeutics Goods Administration (TGA). New applicants must have completed the vaccination course prior to commencement with NSW Health, or provide an approved medical contraindication certificate certifying the worker cannot have any approved COVID-19 vaccines available in NSW.

Acceptable proof of vaccination is the Australian Immunisation Register (AIR) Immunisation History Statement or AIR COVID-19 Digital Certificate. Please provide proof of vaccination if available.

  • Fellowship of The Royal Australian College of General Practitioners (FRACGP) and/ or Australian College of Rural and Remote Medicine (FACRRM)
  • Demonstrated experience in the delivery of General Practitioner services in a complex environment including that of undertaking consultations entirely via telehealth.
  • Proven capacity and/or experience in the provision of effective high quality services, demonstrated through successful clinical practice in an environment appropriate to the requirements of the position including evidence of participation in quality improvement activities relevant to the specialty. Evidence of continuing medical education and self-monitoring/audit practices consistent with the scope of practice requirements of the position.
  • Demonstrated skills and attitudes appropriate as a Senior Medical Practitioner including modelling a high standard of professional behaviour.
  • Demonstrated ability to work effectively and harmoniously with medical and other health services colleagues as part of a multi-disciplinary health care team and demonstrated ability to communicate effectively with patients, colleagues, and staff to work as an effective member of the clinical team across multiple sites
