Understanding the impact of strategic health system reform and translating this into local integrated health plans and initiatives, technology, and evolving models of care, with consideration of rural issues and cross border health seeking behaviors will be critical. The role will build alliances with health and social service organisations to advance innovative approaches to system and service integration. The role will need to ensure key providers are aligned in effort and delivering an improved consumer experience and positive health outcomes for the people of South Eastern NSW at all times, including in preparing and responding to disasters.
Specifically you will be required to have the following skills:
- Forge strong relationships that contribute to the maintenance and growth of health partnership work across Southern NSW working collaboratively to achieve agreed integrated care and disaster preparedness priorities,
- Facilitate cross border ACT initiatives to improve patient journey into and from tertiary care settings,
- Oversee the implementation of ACT &Southern NSW HealthPathways program, through line management of the COORDINARE based team, ensuring a strong partnership approach with Capital Health Network, and contributing to program governance,
- Identify appropriate system integration and initiatives such as service redesign or hospital avoidance strategies aligned with organisational priorities and oversee implementation of new and improved service models or initiatives,
- Support staff reporting to this role in the implementation of key initiatives for example palliative care and dementia care,
- Act as an expert resource for internal team on rural health issues as they relate to developing sustainable service models, implementing change enablers such as use of technology, and workforce planning.
