Clinical Nurse Consultant - Integrated Chronic Care for Aboriginal People Program (ICCAPP)
Mehi Sector - Hunter New England Local Health District

An exciting and rewarding opportunity exists in the Mehi Sector of the Hunter New England Local Health District (HNELHD) for a Clinical Nurse Consultant to join the Integrated Chronic Care for Aboriginal People Program (ICCAPP).

The Mehi Sector includes the towns of Bingara, Boggabri, Moree, Narrabri, Warialda and Wee Waa. Across the Sector, there are a wide range of dining, housing, education and healthcare options, as well as recreational activities, such as bushwalking, artesian spas, camping, and water sports. As the successful candidate, you will work as part of the Mehi Health Service and Aboriginal Health Unit. The position covers the Mehi Sector and will involve regular travel to all of the sites which make up the Sector.

The goal of ICCAPP is to improve access to culturally appropriate chronic care services across the HNELHD for Aboriginal and Torres Strait Islander people aged 15 years and over who have, or who are at risk of cancer, diabetes, cardiac, respiratory, stroke and/or renal disease. The models will include secondary prevention, early intervention, self-management and education programs. As the Clinical Nurse Consultant, you will further support health care providers including Aboriginal Controlled Health Services, and Other Primary Health Care providers through the assessment, case review and development of clinical management plans.

To be successful in this role, you will need to have a high level of clinical skill and consultancy, the ability to role model clinical leadership and a person centred, culturally appropriate philosophy of care. You will need to have the ability to work in partnerships with clients and other services, and you must have effective communication skills.

Hunter New England online learning will be provided to you. Work shadowing and further supported education will also be provided as needed.

Information for Applicants:

All NSW Health workers are required to have completed a primary course of a COVID-19 vaccine which has been approved or recognised by the Therapeutics Goods Administration (TGA). Additionally, Category A workers are required to receive a booster dose three months after completing the primary course of COVID-19 vaccinations. New applicants must have completed the vaccination course prior to commencement with NSW Health, or provide an approved medical contraindication certificate (IM011 immunisation medical exemption form) certifying the worker cannot have any approved COVID-19 vaccines available in NSW.

Acceptable proof of vaccination is the Australian Immunisation Register (AIR) Immunisation History Statement or AIR COVID-19 Digital Certificate. Booster doses are highly recommended for all health care workers who have completed the primary course of COVID-19 vaccinations.
