Job Title: NDIS Case Coordinator

Overview
St Albans Home Caring is seeking a passionate individual to join our team as a NDIS Case Coordinator. The successful candidate will be responsible for conducting support plan assessments, client intake, creating care plans, and liaising with Allied health services. This position offers a chance for a driven individual to demonstrate their abilities in strategic thinking and planning skills. This role is ideal for someone who is bilingual that speaks both Vietnamese and English as majority of our clients are Vietnamese.
Key Responsibilities:
- Coordinate the provision of quality person-centred care according to the client's needs, services, care plans and available funding in accordance with a budget.
- Assist with clinical in-home services include a care plan assessment, medication, incontinence, wound care and work with Allied Health Service to create client care plans and reporting.
- Conduct administrative duties such as data entry, answering calls, and scheduling Allied Health appointments.
- Collaborate with service providers in the wider community to develop a network and ensure comprehensive care options for clients.
- Maintain accurate and up-to-date client records, ensuring confidentiality and professionalism.
- Work autonomously and as part of a small team, taking responsibility for assigned tasks and actively contributing to team meetings and planning sessions.
Qualifications and Skills:
- AHPRA Registered Nurse with current registration status is preferred but not essential.
- Graduate who has studied Social Work, Mental Health or Occupational Therapist are encouraged to apply.
- Comprehensive understanding of the NDIS framework, policies, and standards
- NDISWC Clearance and Police Check
- Own registered vehicle and current driver's license
- Strong organisational and time management skills, with the ability to multitask and prioritise.
- Excellent communication skills, both written and verbal, with the ability to effectively engage with clients and service providers.
- Proficient in using various software applications, such as Microsoft Office suite and database management systems.
