Clarity Support Coordination is an independent NDIS provider who specialise in Support Coordination.
Empowering participants to achieve their life goals and promoting independence, we take a person-centred, co-design approach to planning, implementing, and monitoring NDIS plans and services.
Our commitment is putting our employees and clients at the centre of everything we do and due to continued growth, we are currently seeking a coordinator to join our team. This role would suit an experienced Support Coordinator, LAC or NDIA Planner.
Ensuring collaboration at every step of the journey, you will be responsible for delivering an exceptional Support Coordination service to NDIS participants and working with families and support networks to get the most out of their NDIS plan.
Key responsibilities include but not limited to:
- Delivering exceptional support to your NDIS participants by researching, coordinating and establishing relationships, necessary to support your client and enhance inclusion within the community.
- Recording case notes against participants files
- Maintain a manageable case load and delivering support coordination within the NDIS Funding Frameworks.
- Achieving realistic billable hours against set KPI’s
- Providing regular reports and updates to management on participant issues
- Ongoing review and monitoring of changing NDIS legislation
About you
- You will have experience working as a Support Coordinator/ LAC and have a sound understanding of the NDIS
- Have an empathetic, compassionate and professional working ethic and guarantee a participant focused approach
- Have the ability to work independently and with limited direct supervision in your day-to-day work
- Be trustworthy, respectful and open minded
- Exceptional attention to detail and record management skills
- Highly developed communication and interpersonal skills
- Ability to 'think outside of the box'
- Proficient computer skills and knowledge
- Be resourceful and innovative
- Hold a valid license, have a road worthy car and insurance
The successful candidate will be required to provide clear probity checks including a NDIS screening Check, Working with Children Check prior to commencing employment.
