We are a small family owned and operated company who pride ourself on the high quality support coordination services we offer, and the incredible outcomes for our clients. We are a registered NDIS provider offering Level 2 & 3 Support Coordination who operate from a home office. On offer is a part-time role with the opportunity to move into full-time after our 3 month probation period.
About you
We are a home-based business, and are offering a work-from-home flexible position for a great candidate. You must be able to complete home-visits in the greater Brisbane area. We need someone who is passionate about disability rights and inclusion, who has a proven ability to work independently and is outcomes focused.
Key Responsibilities
- Work with NDIS participants, families, and other stakeholders to implement, monitor and review NDIS plans;
- Support implementation of all supports in the plan, including informal, mainstream and community, as well as funded supports;
- Connecting participants with both formal and informal supports to improve their quality of life;
- Ethical, customer focused practice to provide support to the participants to achieve their goals;
- Timely and accurate record keeping ensuring accountability and service efficiency.
Skills and Experience
- Effective time management and organisational skills, ability to prioritise work and meet deadlines;
- An understanding of the NDIS and its impact on the lives of people with a disability;
- An ability to assess and respond to individual needs;
- The ability to communicate effectively with a diverse range of people to build trust and develop rapport;
- Familiarity with local services, organisations, and other opportunities within the general community
- Ability to use Microsoft Office software
- Proven ability to meet KPI's
You must have a NDIS worker screening clearance check before commencing work. You will also be required to undertake the NDIS Quality and Safeguards Commission’s free online Worker Orientation Module and Induction Module prior to commencing.
Working with Children's Check
You must have a valid Working with Children's Check before you commence work. The check must be an employee check not volunteer.
Mandatory Covid-19 Vaccination
You will be required to comply with the mandatory vaccination requirements as mandated by the Queensland Government.
Job Type: Part-time
Part-time hours: 20-30 per week - Goal to move to full time as quickly as you want to and coordinator be committed to building case load to a full time case load.
Salary: $38.00 per hour
