Role Overview:
Join our NDIS team as a Service Coordinator, driving exceptional customer service for NDIS participants across metropolitan and regional areas. You will coordinate support services, adhering to our organisational policies. Your proactive efforts will propel the implementation of our operational plans and service objectives. Key Responsibilities:
- Ensures person-centred approaches are incorporated into NDIS support plans.
- Implements individual plans with practical and theoretical knowledge.
- Contributes to planning and developing service offerings.
- Ensures customers are supported in resolving their needs, expectations, and goals.
- Communicates comprehensive knowledge of available supports and services.
- Ensures quality of service, effective communication, confidentiality, and diversity awareness.
Skills & Experience:
- Experience in coordinating service provision in the disability sector.
- Significant experience in human services with knowledge of intervention models.
- Experience in working with multi-disciplinary teams.
- Advocated for Participants and mediate conflicts with service providers.
- Coached and assisted with Participant skill development to reduce or eliminate future SC needs.
- Specialist knowledge of disability and support services.
- Ability to advise and facilitate NDIS participants' planning to meet objectives.
- Ability to positively promote services and client issues to organisations, government bodies, and community groups.
Qualifications:
- Tertiary qualification in community services, social work or equivalent and extensive community work and training (essential)
- NDIS Orientation Module (essential)
- Current SA driver’s licence (essential)
- Current Safe Environments for Children and Young People – Through Their Eyes Certificate (essential)
- Current National Police Clearance, DHS Working with Children Check and NDIS Worker Screening Check (essential)
