Your opportunity:
- The Transition Care Program (TCP) is a jointly funded initiative of the Commonwealth Department of Health and Ageing and Queensland Health, offering time-limited and goal oriented flexible rehabilitation care packages to older persons who have had a recent acute or sub-acute hospital admission and live within the Wide Bay Health Service region. It is aimed at older clients, but age alone is not an eligibility criteria.
- Deliver effective rehabilitation occupational therapy clinical services to Transition Care clients:
- maximises client function in activities of daily living,
- maximises functional recovery
- supports timely transition to the Transition Care program
- supports clients' abilities to stay in their own home.

Our offer:
You will receive a competitive remuneration package value up to $9,916.33 p.m., comprising salary rates between $2,719.80 - $3,997.60 p.f., employer contribution to superannuation (up to 12.75%) and annual leave loading (17.5%) (HP3) (Temporary Full Time, 76 hours p.f. from date of appointment up to 25th June 2023 with the possibility of extension).
