About
A hybrid role with time split between clinic, community & telehealth. You will provide high quality, evidence-based interventions, along with tailored support to a population with a range of Musculoskeletal injuries, psychosocial barriers and trauma-related presentations resulting from motor vehicle accidents or workplace injury.
The Role
Responsibility for the management of a caseload of patients ensuring weekly contact, review of exercise program, application of strategies to improve compliance.
Technology (SWORD/Physitrack) - troubleshooting, equipment set-up as required
Face to face and/or TeleHealth clinics to onboard/review patients as part of a multi-disciplinary team. Up to 2 x clinics per week.
Completion of accurate patient notes in line with requirements following clinic reviews and patient phone contact
Booking of new referrals and follow-up appointments in a timely manner
Responsibility for quality control and sending of reports in line with the ARC standards and goals for recovery
Case manager liaison and assisting with claim management - upgrades in capacity, referral for IMC etc
Attending case conferences to facilitate communication between all parties and address any barriers to recovery or return to work/function to achieve a successful outcome
Participate in weekly reviews of ‘flagged files’ to discuss difficult cases with the multi-disciplinary team
Monthly file review with team leaders to promote high quality case management
Monthly file summaries on patients progress / barriers
Liaison with the admin team with regards to: appointment bookings/changes, equipment related concerns
Liaison with the Psych team to discuss Psychological issues/treatment/progress on shared files.
Participation in monthly in-services. Responsibility for running an in-service session on a rotational basis.
Provide support, assistance, coverage for other staff members as directed by Team Leaders/Management
Essential Criteria
Registration as a Physiotherapist with AHPRA/ Exercise Physiologist with ESSA.
Strong interpersonal skills and well-developed negotiation skills within a multidisciplinary and collaborative team.
Experience supporting patients with musculoskeletal conditions to regain function and return to work
High level administrative/computer skills with attention to detail, be proficient in Office 365 Suite other web-based applications (Salesforce, Zoom, etc.)
Desirable Knowledge, Skills and Experience
Experience in motivational and health coaching type environments
Experience supporting patients with musculoskeletal conditions to regain function and return to work
Knowledge of workers compensation
