About the role:
The overall purpose of the Practitioner - Out of Home Care is to provide case management to children or young people within residential care services or programs.
The role will assist to develop participation in positive age-appropriate activities; general physical and emotional wellbeing; engagement in schooling, employment or other training; relevant life skills; and positive connection with family and other significant people.
We are recruiting for a full-time permanent experienced Case Manager/Practitioner to be based at our Gympie office.
About You:
To be successful in this role, you will have the following experience and capabilities within a similar role:
Applied knowledge and understanding of youth justice and child protection frameworks including the Youth Justice Act and Child Protection Act (1999) and other relevant legislation
Understanding of child and adolescent development including impacts of trauma, attachment disorder and grief & loss
Assessment of complex to extreme needs of young people and the implementation of strength based care plans that target areas for development
Ability to respond appropriately in crisis situations; highly developed problem solving, decision making and negotiation skills
Able to work effectively in a team and independently; collaboration with stakeholders and meeting service delivery demands
Strong verbal and written communication skills; report writing and meeting planning
Additionally you must possess:
Current Blue Card
Current Licenced Care Service (LCS)
Current Queensland Driver Licence
Degree in Health, Human Services, Behavioural Science, Social Work, Psychology (this is an essential requirement of the role)
Ability and willingness to travel within the service region
Ability and willingness to work outside of standard business hours and provide on-call support as required
