Overview
The Program Manager GEM Service, Strathalbyn & Districts Health Service is responsible to the BHFLHN Restorative Aged Care Lead and will strategic and operational oversight to the Geriatric Evaluation & Management (GEM) Service at Strathalbyn District Health Service (SDHS).
- Providing oversight to the Quality, Safety and Service Delivery for the GEM cohort of patients.
- Working in collaboration with BHFLHN Patient Flow coordinators or similar to identify patients appropriate for the Strathalbyn GEM Service.
- Coordination and delivery of a GEM education and training program to nurses/carers/allied health team.
- Contributing to the completion of Functional Independence Measurement (FIM) on patients and overseeing the collection of AROC (Australasian Rehabilitation Outcomes Centre) data.
- Performing a liaison function to support positive relationships between Southern Adelaide Local Health Network (SALHN) / Division of Rehabilitation, Aged and Palliative Care and relevant staff within Strathalbyn Hospital / Barossa Hills Fleurieu Local Health Network (BHFLHN).
Employees classified at this level are experts within their area and stream of practice and have a significant degree of autonomy and decision making. They will bring clinical expertise for specified individual patients/clients and/or groups/populations, and/or provide clinical leadership for an area of practice; this being specialist geriatric management.
Various practice models may be used to enact this role, including but not limited to:
- Providing clinical leadership to the GEM Service team including allied health and nurses/midwives.
- Coordination and leadership of projects and/or programs that contribute clinical expertise to improve patient/client/service outcomes.
- Supports an interdisciplinary approach to care.
