Position Overview: As a Registered Nurse specialising in disability community care, you will play a crucial role in enhancing the well-being and quality of life of our clients. You will be responsible for developing and implementing personalised care plans, conducting client visits, managing medications, performing wound assessments, monitoring overall health status, and providing client-focused care. Your role will involve regular check-ins with clients, ensuring their physical and emotional needs are met, and collaborating closely with multidisciplinary teams to optimize client outcomes.
Key Responsibilities:
- Develop comprehensive care plans tailored to each client's individual needs, preferences, and goals.
- Conduct regular client visits to assess health status, monitor progress, and provide hands-on care as needed.
- Manage medication administration, ensuring accuracy, compliance, and adherence to prescribed regimens.
- Perform thorough wound assessments, implement appropriate interventions, and monitor healing progress.
- Monitor and evaluate overall health status, including vital signs, symptoms, and response to treatment.
- Provide client-focused care, emphasizing dignity, autonomy, and respect for individual choices and preferences.
- Collaborate with interdisciplinary teams, including physicians, therapists, and social workers, to coordinate care and optimize outcomes.
- Educate clients, families, and caregivers on health management, medication administration, wound care, and preventive measures.
- Maintain accurate and up-to-date documentation of assessments, care plans, interventions, and outcomes in accordance with regulatory standards and organizational policies.
- Participate in ongoing professional development activities to enhance clinical knowledge, skills, and competencies.
Qualifications:
- Bachelor's degree in Nursing from an accredited institution
- Current unrestricted license to practice as a Registered Nurse (RN) in Adelaide.
- Minimum of 2 years of clinical experience, preferably in community or disability care settings.
- Strong clinical assessment and critical thinking skills, with the ability to recognise changes in health status and respond appropriately.
- Proficiency in developing and implementing individualized care plans based on assessment findings and client goals.
- Experience in medication management, including administration, dosage calculation, and medication reconciliation.
- Competency in wound assessment and management techniques, including wound care protocols and treatment modalities.
- Excellent communication and interpersonal skills, with the ability to build rapport, establish trust, and effectively collaborate with clients, families, and interdisciplinary teams.

- Ability to work independently and as part of a team, demonstrating flexibility, adaptability, and professionalism in a dynamic healthcare environment.
