About The Opportunity
The Sustaining NSW Families Program provides an exciting opportunity for a Registered Nurse, with experience in working with vulnerable families with children from birth to 2 years old, who has / working towards child and family health nursing qualification. It’s a nurse led program. You will be working with other Allied Health Professionals to case manage and provide care to families experiencing vulnerabilities in the Liverpool and Fairfield areas from antenatal to the child 2nd birthday.

What You'll Be Doing
Child and Family Health Nurses (CFHN) working in the Sustaining NSW Families (SNF) program are the front-line service providers. The CFHN in SNF practices at an extended level of nursing, working in a primary health care model, undertaking sustained health home visiting (SHHV) with families who are identified as part of the SAFESTART process as having Level 2 vulnerabilities. The SHHV program works with these vulnerable women and their families, from the antenatal period, potentially until the child is 2 years of age. The (CFHN) is responsible for:

  • Providing best practice evidence-based care to identified vulnerable families using the Sustaining NSW Families program model,
  • Promoting parent-child relationships and optimal child development by working in partnership with parents,
  • The case management of families to ensure they have an effective and integrated plan of care and working in partnership with general practitioners, midwives, perinatal mental health, government and non-government organisations and the members of the SNF multidisciplinary team.

How to Apply
To be considered for this position, please ensure you address the selection criteria as thoroughly as possible.

  1. Current Authority to Practice as a Registered Nurse with Australian Health Practitioners Regulation Agency (AHPRA). Child and Family Health nursing qualifications or commitment to undertake and complete in first 12 months. Demonstrated recent experience as a Registered Nurse in an environment appropriate to the Child and Family Health Nursing Speciality.
  2. Demonstrated ability to provide safe compassionate quality care that is culturally responsive.
  3. Demonstrated ability to effectively communicate and engage with clients/consumers, carers and the multidisciplinary health care team to identify and ensure responsiveness of care needs.
  4. Demonstrated capacity to interact with others in a personable and professional manner and personally resolve conflicts when they arise.
  5. Demonstrated evidence of the use of critical thinking as it relates to the delivery of evidenced based nursing care.
  6. Demonstrated understanding and participation in risk management, work health and safety and quality improvement in the workplace.
  7. Demonstrated ability to work independently and collaboratively as part of a multidisciplinary team.
  8. Current unrestricted Australian drivers licence (P2 Licence Acceptable) subject to obtaining NSW drivers licence within 3 months of appointment.
