About Your Role
Community Transition Aged Care Program (TACP) serves to transition aged care clients safely from hospital back into the community. The TACP clinician, will work collaboratively with the multidisciplinary TACP team across the district with the aim of ensuring clients have equity of service provision and are supported to achieve their goals.
