Permanent CareSpecialist Services provides a permanent care program that place infants, older age children, sibling groups and children with a disability into permanent family arrangements. This role will be focused upon completion of kinship care conversion to permanent care assessments across the Wimmera South West Area. Opportunities to participate in Community applicant assessments, child profile assessments, and linking children to families will also be provided. At times, PCSS also undertake foster care conversion to permanent care assessments.
Placement Coordination Unit
The Placement Coordination Unit is responsible for coordinating all referrals from child protection for placements of children in the funded out of home care (OOHC) system:
- liaising with agencies to determine best placement for children and young peopl
- monitoring and supporting existing placements to ensure optimal placement planning
- coordinating and facilitating links between Child Protection, Agency Performance, community service organisations (CSOs), disability services and housing services as required, to ensure children and youngpeople's day to day care needs are addressed.
