About the role

The Senior Support Coordinator is responsible for providing NDIS participants with high level support in coordinating their plans and negotiating appropriate support and services. They will ensure each participant’s plan is implemented and their goals achieved. They will contribute to the Business Development of the service, connecting with external partners to create referral pathways.

Establish a positive collaborative relationship with the participant and their support network, assisting them to identify, link and coordinate support within the local community, as well as build skills, overcome barriers and achieve goals.

This role is designed to coach and mentor Support Coordinators, providing training and guidance, whilst managing a Level 2 -3 caseload. The Senior Support Coordinator is to support the Manager of Disability Services with the day-to-day operations, financial sustainability and high quality of service delivery.

Provide advice and guidance to participants with various NDIS packages, building their capacity and ability to navigate service systems and developing resilience in the participant’s network
Maintain an understanding of client needs and use this knowledge to plan and provide recommendations on the Disability Service
Liaise with and report as required by NDIA in relation to NDIS participants and their Plans
Navigate, influence and negotiate with public sector and community-based service systems regarding the delivery of appropriate supports in accordance with NDIS Plans
Provide input into the systems to collect information required for reporting and planning
Provide coaching and mentorship to other Support Coordinators, through training and guidance, whilst managing a Level 2 -3 caseload

About You

You have minimum 4 years’ experience in a similar role in community services or case management
You have a Tertiary Qualification in Social Work, Health, Community Services or a relevant field (preferred but not essential)
You have a strong understanding of the NDIS framework
You have experience providing expert advice and consultation to NDIS participants and their families on the changing NDIS environment
You have experience creating increased opportunities for people with a lived experience of a mental health issue to be connected in local
You have experience with facilitating multi-disciplinary coordinated planning where multiple providers work together
You have experience building and maintaining professional, client-focused relationships with stakeholders
You have the ability to build the capacity of NDIS participants and their support network; to understand and navigate service systems

The successful candidate will require National Police clearance prior to appointment (completed by 3Bridges), valid NSW Working With Children Check clearance and Current NSW drivers’ licence.
