THE ROLE
The Commonwealth Home Support Program (CHSP) Service Coordinator is to ensure the management of CHSP services to our clients, their families and carers using a wellness approach, whilst embracing a client centred methodology that demonstrates and promotes Amana Living’s Philosophy of Care and core values.
Key responsibilities
Ensuring coordination of services and level of contact with clients is in accordance with CHSP and Amana Living guidelines and where needed contact appropriate referrals to provide further support services
Acting as a first point of contact for all CHSP referrals within the designated geographical area from the Regional Assessment Service (RAS) to Amana Living clients and their families
Meeting clients and conducting the Health and Safety and environmental checks
Providing telephone and face to face support of clients and carers within the allocated geographical areas, with a focus on wellness and enabling independence as per CHSP guidelines
Responding to client complaints and concerns in a proactive and professional manner
Providing after hours on-call duties according to the rostered schedule
Identifying, initiating and coordinating quality improvement activities in relation to client care including quality audits to ensure compliance
ABOUT YOU
The successful candidate will be highly organised with a sensitive and proactive approach to client care and service delivery, collaborating with the Team Leaders relating to Community Support Workers performance, training requirements and client care.

Strong time management and organisational skills
Highly developed interpersonal and communication skills
Proficient in Microsoft Office applications and Client Management databases
Ability to work under pressure to meet deadlines whilst maintaining a high level of accuracy
Strong problem solving and complaint resolution skills and the ability to take initiative with decision making
Essential Criteria
Evidence of COVID-19 and 2023 Flu vaccinations
Valid ‘C’ Class Drivers License and personal vehicle
Ability to obtain a National Police Clearance (within 6-months validity)
Exposure to Aged Care sector (desirable)
Tertiary qualifications of Certificate IV in Aged Care and/or Community Care (desirable)
Previous experience as a Service Coordinator (desirable)
